Clinical Pharmaceutical Services
Our registered pharmacists will review your medication profile to
prevent overlapping therapies and/or drug interactions.
We also monitor and verify your clinical status and
lab results and make reports to your attending physician.

Skilled Nursing
Every one of HomeChoice's registered nurses has been
specially trained in IV therapy procedures.
Our nurses follow the standards of practice published by the
Intravenous Nurses Society. The HomeChoice staff includes nurses that are board-certified through the
Infusion Nurses Certification Corporation (INCC). These highly trained nurses are experts in treatment, nursing
diagnosis, communication, implementation and evaluation. Our nurses are an excellent resource to all of the
local nursing agencies and are available 24/7.

Your infusion nurse will provide you with the direct IV therapy support and education. This nurse will also work
with other members of your health care team (e.g., physicians, pharmacists, and nutritional experts) to ensure
safe, effective, quality care and prevent the duplication of services among your health care team.

Nutrition Support
If you are receiving enteral or parenteral nutritional therapies, our dieticians will provide comprehensive
nutritional assessment and monitoring. The HomeChoice nutrition staff are Board-certified Nutrition Support
Clinicians with broad expertise in caring for patients in need of enteral (tube feeding) and parenteral
(intravenous feeding) nutrition support.

Supplies and Equipment
HomeChoice will arrange delivery of the necessary supplies and equipment associated with your therapy, such
as infusion pumps and devices, directly to your home. Simply contact your HomeChoice home care
representative. Should you need equipment support, clinical and technical support is available 24 hours a day,
7 days a week.

Good Neighbor Referral Service through Lutheran Senior Services
Good Neighbor caregivers are paid an hourly rate ranging from $9 to $11.50 per hour.
Each independent caregiver undergoes a police background check, is screened through the Family Care
Safety Registry, and is bonded when they begin their association with LSS. Clients are responsible for
determining the suitability, training, or experience of any individual hired.    

Since Good Neighbor Program caregivers are self-employed, the client pays the worker directly for care or
services.  Cash or personal checks are accepted. The client assumes all responsibilities for paying the
required taxes. For information on paying Social Security taxes, call the Social Security Administration at
1.800.772.1213.
For information about paying federal taxes, call the Internal Revenue Service at 1.800.829.1040. A personal
tax consultant should also be contacted.

Handymen either charge hourly or flat rates.  The client bears the responsibility for negotiating the total cost of
the job prior to start of service.

Clients are responsible for determining the suitability, training, or experience of any
individual hired.

Requesting Background Information
Information may be requested in three ways:

The Family Care Safety Registry maintains a toll-free number for persons to request
background screenings on registered caregivers for employment purposes.

The
Employer Background Screening Request form allows employers to submit an
inquiry on multiple employees or prospective employees. Background screening
requests for volunteers must be made by form.  This form may be submitted via FAX or
mail.  

The Family Care Safety Registry Internet Users may request screenings through the
Internet, by clicking on
